WIAA Forms Information

All Athletic Forms are Due Prior To Competition: 
*** If you're unsure about the status of your physical for the upcoming season, please reach out to Mrs. Hartl (hartlca@medford.k12.wi.us) or call the MASH Office at 715-748-5951 ext 4424.

MASH is a member of the WIAA and, therefore, MUST have the following forms signed and turned into the MASH Athletics Office prior to participating in practice at the start of the season:

• A physical every two years performed by a medical doctor
  • every other year an alternate (alt) card must be turned in
• A concussion/cardiac arrest form signed by both athlete and parent
• The Co-Curricular pledge, parent consent and drug testing consent form (found on the "code of conduct" page) signed by both athlete and parent

MAMS is NOT a member of the WIAA.  Therefore, MAMS athletes do NOT have to have a physical, but it is highly recommended that athletes have physicals to make sure they are healthy to participate.
